Tooth transplantation (autotransplantation) is a unique surgical procedure that allows restoration of a lost tooth by moving the patient's own tooth (most often a wisdom tooth or premolar) from one area of the jaw to another. This is a complex microsurgical operation that requires thorough preoperative diagnostics, high surgeon qualification, and precision equipment.

The doctor evaluates the donor tooth (most often a wisdom tooth): its position, condition, and absence of inflammation, as well as the recipient area — bone volume, mucosa, condition of adjacent teeth, and bite.
A CT scan is performed to analyze the shape, length, and stage of development of the donor tooth roots, as well as the volume and density of bone in the transplantation area and proximity to anatomical structures.
Based on the CT scan, the atraumatic tooth extraction, socket formation, fixation method (splinting), and duration of the operation are planned. The treatment plan is explained to the patient.
A strategy for preserving the viability of the periodontal ligament is developed, and the need for further endodontic treatment and monitoring is determined.
The patient is explained the stages of the operation, an anesthesia method (local anesthesia or sedation) is selected, and psychological comfort is ensured.
Postoperative recommendations are provided, a follow-up appointment is scheduled after 7-10 days, and further X-ray monitoring is prescribed to assess the engraftment of the transplanted tooth.

Indications for Tooth Transplantation

Most often, transplantation is used to replace the first permanent molars (6th teeth), which are often destroyed or extracted in childhood and adolescence. The ideal donor is third molars (wisdom teeth) at the stage of incomplete root formation.
In cases of fresh trauma with tooth loss, transplantation is an excellent alternative to implantation, especially when implantation is impossible for medical reasons (incomplete jaw growth, systemic diseases, contraindications to surgical intervention).
In some cases, patients have impacted teeth (most often canines or premolars) that cannot be moved into the dental arch using orthodontic methods due to their incorrect position or ankylosis. Such a tooth can be surgically removed from its pathological position and transplanted into the correct position (for example, into the site of a missing tooth).

During the consultation, the surgeon conducts a thorough examination and a mandatory computed tomography scan to assess the possibility of transplantation: the availability of a suitable donor tooth (most often a wisdom tooth).

Tooth Transplantation with Incomplete Root Formation

This method is used mainly in children and adolescents, when the donor tooth (most often a wisdom tooth or premolar) has an incompletely formed root apex (open apex). This condition is ideal for transplantation, as it provides the best conditions for revascularization (ingrowth of blood vessels) and pulp regeneration.

Transplantation of a Tooth with a Formed Root

This method is used in adult patients when the donor tooth (most often a third molar) has a fully formed root with a closed apex. In such cases, the pulp of the tooth after transplantation, as a rule, does not remain viable, therefore, 2-4 weeks after the operation, after stabilization of the transplant, endodontic treatment (nerve removal and root canal filling) is performed.

Tooth Transposition (Changing Position Within the Same Jaw)

This method is used for orthodontic purposes when a tooth is in an incorrect position (for example, an impacted canine or premolar) and cannot be moved into the dental arch using traditional orthodontic methods. The surgeon carefully isolates the tooth from its pathological position, maximally preserving the periodontal ligament, and forms a new socket for it in the correct location.

Autotransplantation of Third Molars (Wisdom Teeth)

The most common type of transplantation, when an impacted or semi-impacted wisdom tooth is used as a donor to replace a destroyed first or second molar that is scheduled for extraction. 3D Diagnostics and Donor Tooth Selection

A mandatory stage is computed tomography (3D scan), which allows obtaining comprehensive information about the condition of the potential donor tooth (most often a wisdom tooth or premolar): its exact location, length, shape, number of roots, degree of their curvature, stage of root formation (open or closed apex), and the thickness of the bone wall around it.

Surgical Planning and Preparation of the Recipient Socket

Based on the CT data, the surgeon plans the optimal intervention tactic: the extent of surgical access for isolating the donor tooth, the method of its atraumatic extraction with maximum preservation of the periodontal ligament, as well as the formation of a new socket in the recipient area. It is important that the new socket perfectly matches the shape and size of the donor tooth.

Anesthesia and Surgical Intervention (Tooth Harvesting and Transfer)

The operation is performed under local anesthesia with modern drugs (in complex cases or with increased patient anxiety — with the use of sedation). The doctor first carefully isolates the donor tooth, using microsurgical instruments to maximally preserve the periodontal ligament around the root. The extracted tooth is immediately placed in a special nutrient solution to preserve cell viability.

Transplant Fixation and Wound Closure

After placing the tooth in the new socket, the doctor carefully checks its position, occlusal contacts, and stability. Special splints are used for reliable fixation (most often using orthodontic wire and composite material), which connect the transplant to adjacent teeth, ensuring its absolute immobility during the critical period of primary healing.

Postoperative Control and Long-Term Monitoring

The patient receives detailed individual recommendations: mandatory antibiotics (to prevent infectious complications), anti-inflammatory drugs, painkillers, as well as medications to improve microcirculation. Chewing load on the transplant is strictly prohibited. A follow-up visit is scheduled after 7-10 days for suture removal and condition assessment.

Care After Tooth Transplantation

In the first 7-10 days after transplantation, you need to brush your teeth extremely carefully, using only a soft brush and carefully avoiding the area of the transplant, sutures, and fixing splint. It is strictly forbidden to traumatize the surgical area with a toothbrush.
It is strictly forbidden to actively rinse the mouth during the first 3-4 weeks after transplantation, as this can disrupt the process of new blood vessel formation (revascularization), damage the delicate tissues of the forming periodontal ligament, and cause tooth displacement.
Take all medications prescribed by the doctor: antibiotics (mandatory full course to prevent infectious complications and transplant rejection), anti-inflammatory drugs, painkillers, as well as medications to improve blood microcirculation in the transplantation area. Do not miss taking antibiotics, even if your condition has improved.
To reduce significant swelling, pain, and prevent hematoma in the first 24-48 hours after surgery, it is recommended to apply a cold compress to the cheek in the projection of the operation (not on the wound itself!) for 10-15 minutes with breaks of 30-40 minutes.
For the healing period (first 3-4 weeks), completely avoid hard, hot, spicy, and acidic foods. Soft, warm (not hot!) food is recommended: pureed soups, thin porridges, yogurts, smoothies, baby food purees.
Do not miss the follow-up examinations scheduled by the doctor — they are critically important for the success of the transplantation. The first visit is scheduled after 7-10 days for suture removal and condition assessment.

This method is especially relevant for children and adolescents (when implantation is contraindicated due to incomplete jaw growth), as well as for patients who wish to restore a tooth in the most physiological way — with their own tissues.
Is it always possible to find a donor tooth for transplantation?
Unfortunately, not always. The success of transplantation depends on the availability of a suitable donor tooth (most often a wisdom tooth) and its compatibility with the recipient area. During the consultation, we always perform 3D diagnostics to assess the anatomy of the potential donor: length, shape, number of roots, and stage of their formation. A tooth with incomplete root formation (open apex) is considered ideal, as it has the best chances of engraftment and preservation of viable pulp.
Is transplantation painful and how long does the operation take?
Modern anesthesia completely eliminates pain during the operation. You may feel slight vibration or pressure, but no pain. Considering the complexity of the procedure, for patients with increased anxiety we offer sedation (medically induced sleep). The duration of transplantation depends on the complexity of the case and the location of the donor tooth and usually ranges from 60 to 120 minutes. After the operation, when the anesthesia wears off, pain occurs, which is easily controlled with prescribed painkillers.
What are the chances that the tooth will take, and will additional treatment be needed?
With proper patient selection and high-quality procedure performance, the success rate of transplantation reaches 80-90%. The fate of the transplant depends on the stage of root formation: If the root is incompletely formed (open apex) — high chances of maintaining vital pulp; endodontic treatment is not required. If the root is fully formed — 2-4 weeks after transplantation, endodontic treatment (nerve removal and canal filling) is performed to prevent complications. In any case, long-term monitoring is necessary (X-ray at 1, 3, 6, 12 months).
What are the risks of tooth transplantation in the upper and lower jaw?
As with any surgical intervention, there are certain risks. In the lower jaw, the main risk is associated with the proximity of the donor tooth (especially the wisdom tooth) to the inferior alveolar nerve — possible temporary numbness of the lip or chin. In the upper jaw, the risk is associated with proximity to the maxillary sinus. To minimize these risks, we always use 3D planning, which allows us to accurately determine the location of all anatomical structures and choose the safest tactic. Our surgeons have extensive experience in performing such interventions.
How long does the transplant take to heal and when will I be able to chew?
The healing process after transplantation is lengthy and occurs in several stages: Primary soft tissue healing — 7-14 days, sutures are removed after 7-10 days. Initial engraftment (revascularization) — first 4-8 weeks, the tooth remains under a splint, chewing load is strictly prohibited. Complete restoration of the periodontal ligament and bone — from 6 to 12 months. Gradual inclusion of the tooth in chewing is possible only after removal of the splint (usually after 2-3 months) and only with the doctor's permission. Throughout the healing period, it is important to follow recommendations and come for follow-up examinations.
