Gentle Oral Hygiene While Avoiding the Surgical Area

During the first 2–3 days after surgery, teeth should be brushed with a soft toothbrush while carefully avoiding the incision and drainage area to prevent tissue trauma, displacement of the drain, or bleeding.
Instead of active rinsing, which may wash out the protective blood clot or disrupt the healing process, warm oral baths are recommended. Hold an antiseptic solution in the mouth (such as chlorhexidine, miramistin, or a salt-and-soda solution) without intensive rinsing.
All medications prescribed by the doctor should be taken as directed, including antibiotics (to eliminate bacterial infection and prevent complications), as well as anti-inflammatory and pain-relieving medications.
To reduce swelling and pain during the first 12–24 hours after surgery, apply a cold compress to the cheek in the projection of the surgical site (not directly on the wound) for 10–15 minutes with 30–40 minute intervals.
During the healing period (3–5 days), completely avoid hot, spicy, sour, salty, hard, or coarse foods that may irritate the wound or increase inflammation.

What is periostitis, and how is it different from regular toothache?
Periostitis (dental flux) is an acute purulent inflammation of the jaw periosteum that develops as a complication of untreated cavities, pulpitis, or periodontitis. Unlike ordinary toothache, periostitis is accompanied by pronounced swelling of the gums and cheek, redness of the mucosa, elevated body temperature, general weakness, and symptoms of intoxication. The pain from flux is pulsating, intensifies when biting, and may radiate to the ear, temple, or eye.
Can flux be treated without surgery, using only antibiotics?
No, antibiotics alone are ineffective for periostitis. The pus accumulated under the periosteum cannot be absorbed on its own and must be evacuated surgically. Antibiotics are prescribed as an adjunct therapy after incision and drainage to suppress infection and prevent complications, but they do not replace surgery. Delaying the procedure can lead to the spread of pus to surrounding tissues, causing abscesses or cellulitis.
Is it painful to incise flux?
Modern local anesthesia completely eliminates pain during the procedure. We use anesthetic agents that effectively numb even severely inflamed tissues. Patients feel only mild pressure from the instruments, but no pain. After the anesthesia wears off, some discomfort or mild pain may occur, which is easily managed with doctor-prescribed painkillers.
Is it always necessary to extract the tooth in flux cases?
No, not always. If the tooth can be preserved (it is not severely damaged, root canals are passable, and there is no large cyst), the surgeon will perform periosteal incision with drainage, followed by proper endodontic treatment (cleaning and filling of the canals) once the acute inflammation subsides. Tooth extraction is performed only when the tooth is severely damaged, has blocked root canals, a vertical root fracture, or preservation carries a high risk of recurrence.
How long does the wound take to heal after flux incision?
Initial healing after periosteal incision usually takes 5–7 days. Drainage is removed 1–3 days after the procedure, once pus discharge stops. Full tissue recovery can take up to 2–3 weeks. Healing speed depends on the severity of inflammation, promptness of treatment, and the patient’s adherence to postoperative recommendations (taking antibiotics, using antiseptic rinses, following a soft diet).
What should I do if pain increases or swelling reappears after flux incision?
Mild pain and swelling during the first 2–3 days after surgery are normal. However, if pain intensifies, swelling increases, body temperature rises above 38°C, there is a foul odor from the wound, or bleeding worsens, you must contact your doctor immediately. This may indicate a complication (e.g., drainage blockage, pus retention, or infection spread) that requires urgent medical attention.
