Indications for Tooth Root Extraction

If a chronic infection has formed around the root (cyst, granuloma, chronic periodontitis) that periodically flares up with pain, swelling, fistulas, or pus discharge, the root must be removed.
In some cases, the root was originally intended to serve as support for a crown or bridge (for example, after pulp removal).
If a part of the root remained in the socket after a previous tooth extraction (especially under complex anatomical conditions) or if the root broke due to trauma, it must be removed.

Simple Root Extraction Using Forceps or Elevators

This method is used when the root is shallow, clearly visible in the socket, and has no significant curvatures. The dentist carefully severs the periodontal ligament holding the root using forceps or elevators, then gently luxates and removes the root.

Complex Root Extraction with Sectioning (Hemisection)

For multi-rooted teeth (molars of the upper and lower jaw) where the roots are significantly curved, diverge in different directions, or cannot be removed simultaneously, the sectioning method is used. This allows safe removal of each root individually.

Extraction of an Impacted Root with Gum Incision

If the root is completely hidden under the gum and bone (for example, after a failed extraction attempt, incomplete eruption, or trauma), surgical access through an incision in the gum is required to remove the root.

Apicoectomy (Root Tip Resection) While Preserving the Tooth

In cases where the root is retained but a cyst, granuloma, or chronic infection focus is present at the root tip that does not respond to conservative treatment, a root tip resection (apicoectomy) is performed to preserve the tooth.

Diagnosis and 3D Planning

The dentist conducts a thorough examination and mandatory radiological imaging (periapical X-ray or CT scan) to assess the root’s condition: number, shape, length, degree of curvature, thickness of the bone walls, and proximity to important anatomical structures (mandibular nerve, maxillary sinus, mental foramen).

Determining the Extraction Method

Based on the diagnostic data, the surgeon selects the optimal approach: simple extraction using forceps or elevators (if the root is accessible), complex extraction with root sectioning (for multi-rooted teeth), or surgical extraction with a gum incision (for impacted or deeply located roots).

Anesthesia and Surgical Intervention

The procedure is performed under local anesthesia with modern agents. For complex cases or patients with high anxiety, sedation may be applied.

Socket Revision and Bone Preservation

After extraction, the dentist carefully inspects the socket, removes granulation tissue and any fragments, and irrigates with antiseptic solutions. If a cyst or granuloma is present, curettage (scraping) of pathological tissue is performed.

Postoperative Care and Preparation for Implantation

The patient receives detailed personalized instructions: pain management and antibiotics (if needed), antiseptic mouth rinses (without vigorous rinsing!), hygiene rules, diet recommendations (soft, warm foods), and limitations on physical activity.

Gentle Oral Hygiene While Avoiding the Surgical Area

In the first 3–4 days after surgery, brush your teeth gently with a soft toothbrush, carefully avoiding the extraction site to prevent injury to the healing tissues, protect the blood clot, and avoid triggering bleeding.
Vigorous rinsing of the mouth is strictly prohibited during the first days after root extraction, as it can wash out the protective blood clot, expose the bone wound, and lead to alveolitis (“dry socket”).
Take all medications prescribed by your dentist: antibiotics (to prevent infectious complications, especially if the extraction was complex or performed amid inflammation), anti-inflammatory drugs, and painkillers.
To reduce swelling, pain, and prevent bruising during the first 12–24 hours after surgery, apply a cold compress to the cheek over the socket (not directly on the wound!).
During the healing period (5–7 days), completely avoid hot, spicy, acidic, salty, hard, or coarse foods that may injure the socket or worsen inflammation.
Do not miss the follow-up appointment scheduled by your dentist (usually within 5–7 days).

Why shouldn’t a tooth root be left in the jaw if it doesn’t hurt?
Even if the root is painless, it is a potential source of chronic infection. Cysts or granulomas can form around it, gradually destroying the bone tissue. Chronic inflammation may flare up at any time (due to lowered immunity, cold exposure, or stress) and cause acute pain, swelling, abscess, or phlegmon. A retained root can also interfere with proper socket healing and may lead to bone atrophy, complicating future implant placement.
Is it always necessary to remove a root if the tooth is damaged?
Yes, in most cases the root of a damaged tooth must be removed. Exceptions may include cases where the root is planned to be used for prosthetics (e.g., as a post for a crown). However, for this, the root must be healthy, free of inflammation, with well-filled canals and sufficient length. If the root has cracks, perforations, signs of inflammation, or cannot reliably support a prosthesis, it must be extracted.
Does root extraction hurt?
Modern local anesthesia completely eliminates pain during the procedure. The doctor uses agents that provide deep anesthesia even when working with bone tissue. The patient may feel slight pressure or vibration but no pain. For complex procedures (e.g., removal of impacted roots, work near nerves) or heightened anxiety, we offer sedation for complete relaxation. Postoperative pain is managed with prescribed analgesics.
How long does root extraction take?
The duration depends on the complexity of the case. Simple extraction of a well-visible, straight root takes 15–30 minutes. Complex extraction with sectioning of roots in multi-rooted teeth may take 40–60 minutes. If the root is impacted (completely covered by gum and bone), additional time may be needed for soft tissue incision and bone removal. The doctor informs the patient of the estimated duration during the planning stage.
What risks are associated with extracting roots in the lower jaw?
The main challenge is the proximity to the mandibular canal, which houses a nerve. Careless actions may damage the nerve, causing temporary or permanent numbness of the lip, chin, and gums. To minimize this risk, we always perform a CT scan to precisely determine the root’s location relative to the nerve. There is also a rare risk of mandibular fracture when removing large roots, typically only if the bone is very thin.
When can an implant be placed after root extraction?
The timing depends on the condition of the bone and presence of inflammation. Two options are possible:

Immediate implantation — the implant is placed in the socket immediately after root extraction. This is possible if there is no acute inflammation, there is sufficient bone tissue, and primary stability of the implant can be achieved.

Delayed implantation — the implant is placed 3–6 months after complete bone healing. This option is chosen in cases of chronic inflammation, cysts, significant bone loss, or if prior bone grafting is needed. After root extraction, the doctor will discuss the optimal timing for implantation with you.
