Dental cyst removal (cystectomy, apicoectomy) is a surgical procedure aimed at eliminating a pathological lesion in the bone tissue while preserving the natural tooth whenever possible. The procedure requires thorough preoperative diagnostics, a high level of clinical expertise, and modern dental equipment.
The doctor performs an examination, assesses the condition of the oral mucosa, the presence of inflammation, the degree of eruption of the impacted tooth, signs of pericoronitis, the condition of adjacent teeth, and surgical accessibility.

After the procedure, medications are prescribed, and guidance is provided on oral hygiene, diet, and daily routine. A follow-up appointment is mandatory within 5–7 days.

Indications for Dental Cyst Removal
A dental cyst tends to gradually increase in size, even if it does not cause discomfort to the patient. As it grows, it destroys bone tissue, replacing it with pathological tissue. When it reaches a significant size (over 1–1.5 cm), there is a risk of pathological jaw fracture even under minor stress.
A cyst can become infected, especially when immunity is reduced, in cases of hypothermia, or in the presence of other infection sites. An exacerbation is accompanied by severe throbbing pain, swelling of the gums and cheek, fever, general weakness, and the formation of a fistula with purulent discharge.
If the cyst does not respond to conservative treatment (repeated endodontic therapy with temporary canal fillings using medicated pastes is ineffective, the cyst does not shrink, or continues to grow), surgical intervention is indicated.

Types of Surgical Procedures for Dental Cyst Removal
Apicoectomy with Cyst Removal
This is the most common tooth-preserving procedure, used for small to medium-sized cysts (up to 1.5 cm) at the root tip. Through a minimal gum incision (up to 5 mm), the surgeon gains access to the root tip, removes it along with the cyst, carefully scrapes out the pathological tissue, and irrigates the cavity with antiseptics.
Cystectomy (Complete Cyst Removal)
This is a radical procedure in which the cyst is completely removed along with its capsule. The operation is performed for medium to large cysts (over 1.5–2 cm), as well as for cysts with complex structures or those located in close proximity to critical anatomical structures.
Cystotomy (Partial Cyst Removal)
This method is used for very large cysts (over 3–4 cm) that are adjacent to vital structures (mandibular nerve, maxillary sinus, nasal cavity) or for cysts that have extended into the maxillary sinus. The surgeon removes only the front wall of the cyst and its contents, while the inner lining is either connected to the oral cavity (creating an artificial pouch) or drained.
Hemisection and Root Amputation
This technique is used on multi-rooted teeth (molars of the upper and lower jaw) when the cyst affects only one root while the other roots remain healthy. The surgeon removes the affected root along with the adjacent cyst and part of the tooth crown (hemisection) or only the root (root amputation), preserving the healthy portion of the tooth.

Is it always necessary to remove a cyst surgically, or can it be treated conservatively (without surgery)?
Small cysts (up to 5–8 mm) can sometimes be treated conservatively through root canal therapy. The dentist reopens the canals, thoroughly irrigates them with antiseptics, and places special medicated pastes based on calcium hydroxide, which stimulate bone regeneration. This procedure may take several months and requires regular radiographic monitoring. If the cyst does not shrink, is large (over 1–1.5 cm), complicated by inflammation, or causes pain, surgical removal is indicated. The decision is made individually after 3D diagnostics.
Is dental cyst removal painful?
The procedure is performed under local anesthesia with modern agents, so there is no pain during the surgery. You may feel slight pressure or vibration, but not pain. For complex or extensive procedures (large cysts, work near nerves or sinuses) or in patients with high anxiety, we offer sedation (medicated sleep), allowing the patient to be completely relaxed. Postoperative pain is easily managed with prescribed painkillers.
How long does a dental cyst removal take?
The duration of the procedure depends on the size and location of the cyst, as well as the chosen surgical technique. An apicoectomy for a small cyst takes 30–50 minutes. A cystectomy (complete removal) of a medium or large cyst with osteoplasty can take 60–90 minutes. For cystotomy or hemisection, the time also varies depending on complexity. The surgeon always informs the patient of the estimated duration during the planning stage based on the CT scan.
What are the possible risks of dental cyst removal in the upper or lower jaw?
In the lower jaw, the main risk is the proximity of the cyst to the mandibular canal containing the nerve. If the nerve is affected, temporary numbness of the lip and chin may occur. To minimize this risk, we always perform a CT scan and use microsurgical techniques.
In the upper jaw, the cyst may be adjacent to or extend into the maxillary sinus. The risk here is perforation of the sinus floor. In such cases, the surgery is carefully planned, and if a perforation occurs, we immediately perform defect repair. Modern diagnostics and the surgeons’ experience help minimize these risks.
Is it possible to preserve a tooth if there is a large cyst at the root?
Yes, in most cases, the tooth can be preserved even with large cysts. Modern techniques — such as cystectomy with apicoectomy or hemisection — allow the removal of the pathological formation, elimination of the infection source, and preservation of the tooth’s functionality. The decision to preserve the tooth is made after thorough analysis of the CT scan, assessing the condition of the root, its canals, the volume of surrounding bone, and proximity to vital structures. If the tooth is severely damaged or preservation is not feasible, the tooth is removed along with the cyst, followed by implantation.
How long does bone take to heal after cyst removal?
Primary healing of the soft tissues (gums) occurs within 7–14 days, with sutures removed after 5–7 days. Bone regeneration in the cyst area is a longer process. The first signs of bone healing are visible on radiographs after 2–3 months. Complete bone restoration, especially when osteoplastic materials are used, takes 6–12 months. Therefore, we schedule follow-up radiographs at 3, 6, and 12 months to monitor healing, assess material integration, and detect any potential recurrence in time. The rate of healing depends on the size of the cyst, the patient’s age, adherence to postoperative instructions, and overall health.
