Based on the CT scan, an atraumatic extraction technique is chosen to preserve bone, with tooth sectioning planned if needed to reduce trauma.
A strategy for bone tissue preservation is developed: immediate implantation or socket filling with osteoplastic material in preparation for delayed implantation.
The patient is explained the course and duration of the operation, an anesthesia method (local anesthesia or sedation) is selected, and questions are answered.
Medications are prescribed, postoperative recommendations are provided, and follow-up visits are scheduled to assess healing and bone tissue restoration.

Indications for Tooth Extraction Before Implantation

If a tooth is destroyed by caries or trauma to such an extent that it cannot be restored using an inlay, crown, or post-core restoration, further preservation of such a tooth is impractical.
If there is a large inflammatory focus (cyst, granuloma) at the root apex that does not respond to conservative treatment or periodically exacerbates, such a tooth is a source of chronic infection.
Extraction before implantation is indicated in the presence of intractable anatomical problems: perforation of the root wall in the middle or upper third, longitudinal root fracture, or a tooth crack that extends deep under the gum.

Atraumatic Simple Extraction (Closed)

This method is used when the crown part of the tooth is well preserved or slightly destroyed, and the roots have a simple shape without curvature. Under local anesthesia, the doctor carefully separates the circular ligament of the tooth with special instruments (luxators), after which the tooth is dislocated and removed as gently as possible with forceps.

Atraumatic Extraction with Tooth Sectioning (Closed/Open)

In cases of complex root anatomy (curved, branched, divergent), multi-rooted teeth, or a significantly destroyed crown, the method of sectioning the tooth into fragments is used. The surgeon carefully divides the tooth at the root bifurcation site into separate parts using a surgical bur, which are then removed separately through minimal access.

Complex Extraction with Flap Reflection (Open)

This method is used for deep tooth destruction under the gums, suspected root fracture, or for impacted teeth (for example, wisdom teeth) that are planned to be replaced with implants. The doctor makes an incision in the soft tissues, reflects a mucoperiosteal flap, opening access to the bone and roots.

Extraction with Immediate Implantation

In favorable clinical situations — with sufficient bone volume around the tooth, absence of acute purulent inflammation — extraction and implant placement in one visit is possible. After atraumatic tooth extraction, the doctor forms a bed in the bone tissue directly through the existing socket or next to it and immediately places the implant.

3D Diagnostics and Bone Tissue Assessment

A mandatory stage is computed tomography (3D scan), which allows obtaining comprehensive information about the condition of the problematic tooth and surrounding tissues: the exact location of the roots, their shape, number, degree of curvature, and the presence of cysts or granulomas.

Determining the Extraction Tactic and Implantation Plan

Based on the CT data, the implant surgeon chooses the optimal intervention tactic: the extraction method (simple, with sectioning, with flap reflection), the need for immediate implantation (if bone volume permits and there is no inflammation) or delayed implantation.

Anesthesia and Surgical Tooth Extraction

The operation is performed under local anesthesia with modern drugs (in complex cases or with increased patient anxiety — with the use of sedation). The doctor performs an atraumatic tooth extraction using special techniques: careful separation of ligaments, minimal loosening.

Revision, Sanitation, and Bone Tissue Preservation

After tooth extraction, the doctor carefully examines the socket, removes granulation tissue and fragments, and rinses it with antiseptic solutions. If immediate implantation is not planned, to preserve bone volume and prevent atrophy, the socket is filled with osteoplastic material (bone substitute) and, if necessary, covered with a protective membrane.

Postoperative Control and Further Implantation Plan

The patient receives detailed individual recommendations: mandatory antibiotics (to prevent infectious complications), painkillers, antiseptic baths (without active rinsing!), hygiene rules in the surgical area, diet (soft warm food), cold on the cheek on the first day, and limitation of physical activity.

Care After Tooth Extraction Before Implantation

For the first 3–4 days, brush with a soft toothbrush, avoiding the wound and sutures to prevent bleeding or damage. Clean adjacent teeth gently, especially if bone graft material was placed.
Do not rinse vigorously, as this may dislodge the blood clot or graft. Instead, use warm antiseptic mouth baths (e.g., chlorhexidine or chamomile) 3–4 times daily starting the day after surgery.
Take all prescribed medications, including a full course of antibiotics, to prevent infection and support proper bone healing.
During the first 24 hours, apply a cold compress to the cheek (not directly on the wound) for 10–15 minutes with breaks. Swelling may peak on days 2–3 and is a normal response.
For 7–10 days, avoid hot, hard, spicy, or acidic foods. Choose soft, warm meals and chew on the opposite side. Do not use a straw.
Attend your follow-up visit (usually after 5–7 days) for suture removal and healing assessment. Implant placement is typically planned after 3–4 months, following complete bone healing and X-ray control.

Is it mandatory to extract the tooth, or can it be saved?
The decision to extract is made only when the tooth is considered hopeless and cannot be saved. This occurs with crown destruction of more than 50-70%, longitudinal root fracture, large cysts that do not respond to treatment, or root perforation. During the consultation, we always perform 3D diagnostics to assess all possibilities for saving the tooth. If there is no chance, we offer an optimal extraction plan with preparation for implantation, so you do not waste time and money on hopeless treatment.
Is it painful to extract a tooth and how long does the operation take?
Modern anesthesia completely eliminates pain during the operation. You may feel slight vibration or pressure, but no pain. For patients with increased anxiety, we offer sedation (medically induced sleep). The duration of extraction depends on the complexity: a simple extraction takes 15-30 minutes, a complex one (with tooth sectioning, wisdom tooth removal) — from 40 to 90 minutes. After the operation, when the anesthesia wears off, pain occurs, which is easily controlled with prescribed painkillers.
When can an implant be placed after tooth extraction?
This depends on the clinical situation. Three options are possible: Immediate implantation — the implant is placed directly into the socket of the extracted tooth during the same visit. Early implantation — after 1-2 months, following soft tissue healing. Delayed implantation — after 3-6 months, if there was significant infection or bone grafting is needed. The decision is made individually based on CT data and the condition of the tissues after extraction.
Is it necessary to fill the socket with bone material after extraction?
In most cases, we recommend doing this, especially if implantation is planned. After tooth extraction, the bone begins to atrophy (decrease in volume). If the socket is not filled, after 3-6 months there may not be enough bone for stable implant fixation, and an additional operation — bone grafting — will be required. Filling the socket with osteoplastic material allows preserving bone volume and creating ideal conditions for the future implant.
What are the risks when extracting teeth in the upper and lower jaw?
In the lower jaw, the main risk is the proximity of the roots to the inferior alveolar nerve. If damaged, temporary numbness of the lip or chin is possible. To minimize the risk, we always use 3D planning. In the upper jaw, the risk is associated with the proximity of the maxillary sinus: perforation of its floor is possible, especially when extracting chewing teeth. At BIOCLINIC, we perform such interventions with mandatory monitoring of sinus integrity and, if necessary, with simultaneous perforation repair.
How long does the socket take to heal after extraction, and when can I chew?
Primary healing of soft tissues (gums) occurs in 7-14 days, sutures are removed after 5-7 days. Swelling and discomfort may last 3-5 days. Complete restoration of bone tissue in the socket takes from 3 to 6 months. You can carefully chew on the extraction side after 1-2 weeks, avoiding hard food. However, full loading on the bone in this area will only be possible after the implant is placed and fully integrated. Throughout the healing period, it is important to follow the doctor's recommendations and come for follow-up examinations.
